Confined Space Entry Program

Norman Stein & Associates confined space program is designed to ensure the safety of personnel required to enter and conduct work in confined spaces.

Our crew will conduct a pre-job briefing with all personnel involved in the confined space entry which will include:
  • A review of the scope of work to be completed
  • Potential hazards
  • Means and methods to control the hazards
  • Atmospheric testing results
  • Communication method to be used between entrant(s) and attendant(s)
  • Emergency rescue procedures
We begin by completing a Pre Job Hazard Assessment prior to the beginning of each job/task. All potential hazards must be identified, evaluated and controlled.

Our crew also completes an On-Site Safety Inspection Checklist prior to the commencement of any work performed at the customer’s plant.

The confined space must be tested for the acceptable atmospheric conditions with a properly calibrated instrument. N.S. & A. will bring our own currently calibrated oxygen monitors on site. Entry into the confined space is not allowed until acceptable atmospheric conditions have been met.

Confined Space Entry General Requirements checklist will be completed and signed by all personnel involved in the entry prior to commencement of entry into vessel. Confined Space Entry Permits will be maintained and posted outside the entrance while entry operations are ongoing.

All entrants must wear a safety harness with a retrieval line attached to the harness at all times. For entries with a vertical drop of five feet or more from the opening, a mechanical retrieval device must be available and in place at the point of entry, and attached to the entrant in order to conduct a non-entry rescue. This device will be designed and labeled as a means of retrieval for life loads. Chain falls, hoists, etc. are prohibited due to not being designed for retrieving life loads.

If the confined space atmosphere cannot be monitored continuously (i.e. due to waterblasting, sandblasting, etc. in the space) the entrants must use supplied air. Respirator training and fit testing is conducted annually, and Pulmonary Function Tests are completed as directed by the medical professional.

All entrants will use their personal protective equipment as needed to perform assigned tasks.

Lockout/Tagout is performed for the control of hazardous energy. This measure is implemented to minimize those hazards to ensure the safety of all personnel in the surrounding area.

N.S. & A. will provide all arrest equipment for adequate fall protection, including harnesses, lifelines, lanyards, and straps. The purpose of this safety procedure is to establish safe use of ladders and scaffolding.

Our crew will complete a Hot Work Permit for any activity which results in sparks, fire, or hot material which has the potential to cause fires or explosions.

We conduct a Toolbox Safety Meeting weekly while on job site.

Confined space program training is conducted annually for all employees.
